Summary
Person in this position will be responsible for the case management of a large caseload of participants in program.
Responsibilities
* Case management of a large caseload of participants;
* Make follow up contacts with consumers in the program;
* Heavy documentation;
* Coordination of various services throughout several agencies;
* Attend scheduled meetings within the community;
* Travel to community based providers;
* Embrace and embody the mission, vision, guiding principles, clinical vision and goals of WestCare Foundation; and
* Perform any other duties as assigned.
Ideal Candidate
* Two (2) years case management experience;
* Excellent communication skills;
* Basic computer skills;
* Good documentation skills;
* Be able to multi-task; and
* CPR Certification, First Aid Certification and an Annual Tuberculosis test
**Education:**
* AA Degree is required; and
* Graduation from an accredited four-year college with a degree in Psychology, Social Work, Rehabilitation Counseling or a related field is preferred.

Work is primarily performed in an office setting; Some travel is required; and Some outdoor activities may be required.
