Nurse Practitioner, Hospice

Nurse Practitioner, working Full Time, to provide advanced nursing care to hospice patients. Work under the supervision of the Medical Director(s) to function as individual patient's attending physician, and/or make face to face visits as required by Medicare.

• Work with Medical Director(s) to develop and maintain written protocols for providing care as required by the Board of Registered Nursing.
• Conduct comprehensive interviews and physical assessment of patients in order to develop accurate medical histories and identify factors that indicate terminal status.
• Provide palliative care to individual patients based on written protocols.
• Share in providing 24-hour, seven days a week on call medical coverage for Hospice patients.
• Function as an expert resource for staff providing consultation regarding assessment, patient care management, and palliative care precepts.
• Perform all other duties as assigned.

• Current Nurse Practitioner license with adult, geriatric. or family emphasis.
• Experience in hospice or palliative care preferred.
